Today's episode HR Huffing Stuff Last century before the existence of the Internet, people would have to figure things out by going to the library or just by trial and error. No matter what the topic was, you had to put some effort into it to find the information out. And I had friends that would spend their day researching and trying to figure out ways to get drunk or high. I've talked about them before on episodes like Foraging Misadventures when they were trying to lick toads, and the Listerine Drinker on the episode Minty Fresh. And today I will be talking about some of them again. The question I always get asked is why did I hang out with people like this if I didn't do drugs or drink and the answer is. In school I didn't fit in with any particular group completely. But I favored the non judgmentalness of the heavy metal enthusiasts and the stoners. And these are the groups of friends that were constantly trying to get high. Why did I hang out with them? Well, people often thought I was high because I was always tired. I fell asleep a lot in class and my eyes always looked sleepy. And people still think I look high to this day. The reality is I always got up every day at 4am to throw papers with my dad. And I also often didn't go to bed before 11pm because I was working on schoolwork. So on average I would get about five hours of sleep. I was constantly functioning at a near exhaustion level, which resulted in a very tired, disheveled look. But I wasn't high. But because of this perception, I often hung out with the stoners and the heavy metal enthusiasts. They were nice people and I didn't judge them and they didn't hassle me about being sleepy in class. On one evening they even asked me if I wanted to participate in smoking marijuana and drinking beer. I declined and the response was cool. More for us that evening ended with me completely sober, crammed in the back of an old Buick, no seatbelt on, sitting with four other people on a bench seat as the inebriated driver and the drunk front seat passenger tried to navigate to find the local pool hall. The copper cue to be exact. Conversations were always rambling and non linear. But I really had nothing better to do on a Saturday night. But I really should have been driving. In my hometown of Wichita, Kansas in the late 1980s, there was not a lot of things to do. You could work, go to school, play sports and do other things. I often participated in the other things and they're the ones that I enjoyed the most. But we won't go into those. The friends I had that liked to do drugs started with marijuana. One of them grew it in a secret spot in his dad's field. But they got tired of marijuana. It didn't really give them the effect they wanted, probably because it wasn't very potent. They wanted to branch out. Cocaine, lsd, pcp, mushrooms, heroin, methamphetamine were not readily available back then. Plus you would have to take active measures to find them. And possession of these drugs could result in serious jail time, which was always a deterring factor. They instead tried things that were readily available in their house. They would spend hours every week looking at ingredients and labels of things in the kitchen, bathroom and garage, trying to figure out if there was something in the product that could mess them up. Most of the time it would make them sick or give them a headache. When they found something that worked, they would let the group know. And I never participated in any of their findings. Some of my other friends did though. Why were they always trying to figure out a new way to get high? It's hard to say. They were pretty intelligent people. The problem was they didn't use their intelligence to do better in school or music or art. Art or something constructive. They instead just wanted to find out whatever they could to help them escape the boredom of everyday life. They had a few successes. One was the intoxicating effects of Listerine and the other one was they huffed propane. Huffing refers to intentionally inhaling chemical vapors from household or industrial products. They tried huffing a number of different items but stuck with propane. Not the wisest of chemicals to inhale. Its effect is mainly lightheadedness and it causes this by interfering with neuroreceptors and displacing oxygen. So essentially all the high is is your brain cells dying from lack of oxygen. Inhaling fumes was not an original idea. In fact, this practice to get high has been around for at least 200 years and today I'm going to talk about some cases of other people who figured out huffing chemicals, whatever they are, can get you high and sometimes can cause you to die. So let's get started. There are more than a thousand household products that are inhaled for their mind altering effects. They're convenient, inexpensive, easy to hide and of course legal. You can get all you want at any store, depending on what you choose to get high with. The majority of the inhalants affect the body in similar ways. The chemical enters the bloodstream quickly through the large absorptive surface of the lungs and and heads to the central nervous system where it acts as a central nervous system depressant by interfering with neurotransmitter activity and enhancing the depressive effects of gamma aminobutyric acid receptors and inhibiting the N methyl d aspartate receptors which causes feelings of excitement and disorientation and euphoria followed by drowsiness, lightheadedness, decreased inhibitions and agitation. The inhalants that are abused belong to four broad categories and these are volatile solvents, which includes paint thinners, gas glues and felt tip markers, which was another thing my friends tried to get high with aerosols which include spray paints, deodorants and hair sprays. Gases like ether and nitrous oxide and nitrates, which are known as poppers. And these act differently than the rest. They cause vasodilation and muscular relaxation and are often often used for sexual activity and include substances like amyl nitrate and butyl nitrate. Statistically, adolescents and teenagers are often the individuals abusing inhalants because they are easy to acquire. But the cases I have seen of people dying from inhalants are typically adults. The ones we see most in the morgue are the ones that huff spray paint and canned air. Nitrous oxide and freon spray paints and canned air are the most common, with canned air becoming more common lately. But spray paint is the classic one. Its abusers have colorful nicknames, including chromers. They spray the spray paint into a bag and then inhale, which is called bagging. Or they spray it into a rag and huff it that way. The high is caused by a chemical within the spray paint called toluene. It's in highest concentrations in the silver and gold spray paints. Toluene causes its effects like all the other inhalants and has similar outcomes. The high of toluene is intense but short lived, which makes the huffers huff multiple times from their bag over the course of hours or even a day. It attacks the nervous system and may cause permanent damage in a single exposure. Heavy use causes drowsiness, fatigue and confusion that can last minutes or hours. It can also cause irritation in the upper respiratory tract. And chronic use can cause clinical depression. Huffering spray paint can also cause a syndrome known as spray head syndrome, where the individual acts bizarre with disinhibition and aggressive behavior. At autopsy, the individuals often have silver or gold spray paint around their hands or about their face, going into their nostrils and on their tongue. Internally, there might be slight brain atrophy and pulmonary edema or congestion, meaning fluid on the lungs, but no other real findings. Special testing has to be done to find it on a toxicological examination which shows toluene present in the blood. Canned air is commonly used as an air duster for cleaning electronics. But it does not contain ordinary air. Instead, it contains hydrofluorocarbons, specifically 1:1 difluoroethane. The effects of canned air are again the same as other inhalants. But the canned air cases that I have seen have very unusual scene findings. And what you find are multiple cans of canned air everywhere, on the couch, in the bed. It just depends on where they get lit up. I've seen several cans in the bathroom or on people's cars, on the floor of the car. In the glove box, in the center console, in the kitchen, and even in their workplace, they don't even try and hide it. It's almost like they just don't think. People will think they're using it to get high. When they die, they may be holding the can in their hand with the small straw coming from the can and sticking in their nose. At autopsy, there are no real direct findings. And again, the diagnosis is all based on toxicologic findings. And what you get is 11 difluoroethane present in the blood. The next common inhalant that we see generally at the morgue is nitrous oxide, or laughing gas. Nitrous oxide is a colorless, odorless gas used for medical anesthesia and pain relief. It causes brief euphoria, giggling, floating sensations, dizziness, visual and auditory hallucinations, and inappropriate behavior. Nitrous oxide abuse is somewhat common in the population, but amongst medical and dental professionals, it is a little more common. And One survey showed 20% of medical and dental students have actually tried it. For dentists, it ranks after alcohol dependence, prescription and opiate abuse, and other illegal drugs, comprising 5% of dentists who abuse drugs. In my experience of medical professionals who die of overdoses, nurses are the most common, followed by doctors, particular anesthesiologists, and then dentists. But for inhalant abuse, all I have ever seen for medical professionals has been dentists. In one particular case, the dentist would de stress after work by inhaling nitrous oxide to relax. After everyone else had left, he would huff a little nitrous oxide to take the edge off of his day before he went home. Over time, he needed more and more to settle down. Also, the nitrous abuse was beginning to cause problems, specifically neuropsychiatric symptoms, which we'll go into more later. On the day he died, it had been evidently a particularly stressful day. Probably he had to do one too many root canals or some other detailed procedure. After work, he slipped on the oxygen mask, leaned back in his dental chair and turned on the nitrous tank. And that was how he was found by his office manager the next day. She opened up the door to the practice, came in like she always did, turned on the lights, and found him reclined in the dental chair, oxygen mask still in place, nitrous tank completely empty. 911 was called, and he was pronounced dead at the scene. His autopsy was done the next day, and he was found to have pulmonary congestion and edema. And toxicology testing had to be performed to determine that he had died from his nitrous oxide intoxication and not another drug. The thing is, nitrous oxide isn't always detected in routine toxicologic testing because occasionally it can disappear quickly after death. But autopsy toxicologic screens can identify nitrous oxide in biological samples such as blood, urine, brain, liver, lung and tracheal airway gases. And detection can even be possible even up to one month after death. In some cases, this persistence is due to residual gases in tissues and organs, though concentrations decline over time. But even if nitrous oxide doesn't show up on toxicologic testing, you can still diagnose it by circumstantial evidence. With the dentist, we were able to find nitrous oxide in his blood. That rolled Quesadilla. Not a rolled quesadilla anymore. Now it's a sauce shovel. Other common nitrous oxide cases we see are high school and college students with plastic bags over their heads and multiple cans of Redi Whip at their side. It doesn't have to be Redi Whip. It can be other variants of aerosol cream. But the key factor is it has to be an aerosol dispensed whipped cream made with real dairy cream. I'm sure you've seen it in the grocery store or used it yourself as a topping on desserts. The propellant associated with whipped cream is Actually, nitrous oxide. The reason for this is nitrous oxide, when injected in cream under pressure, dissolves into the fat content of the cream. Upon release through the nozzle, the gas rapidly expands, incorporating air and forming tiny bubbles. It gives whipped cream its characteristics, fluffy texture and stable structure. It was chosen over carbon dioxide, which is typically used in carbonated beverages, because carbon dioxide causes a sour taste, whereas nitrous oxide is slightly sweet. Carbon dioxide doesn't create the thick, pillowy texture like nitrous oxide does, and it's less stable. Also, nitrous oxide has a bacteriostatic property, meaning it inhibits the growth of bacteria, which carbon dioxide does not. People have to buy multiple aerosol whipped cream cans and empty them upright into a balloon or into a plastic bag over their head, inhaling it as they go to get high. The other thing that can be used is whipped cream dispensers from a restaurant store or Amazon. These use small nitrous containers that are small cylinders that are a couple of inches long containing 8 grams of nitrous oxide. A device also can be purchased, called a cracker, from a drug paraphernalia store that can empty a nitrous oxide cartridge into a balloon for the ease of inhaling. These cartridges have a nickname, and they're called whippets with nitrous oxide. Most people die during acute use, and this is because of asphyxiation. Nitrous oxide, much like propane, displaces oxygen, and if only nitrous oxide is available and no new oxygen enters the body, the individual dies from lack of oxygen getting to the brain. With whipped cream huffing, it is also because the individual passes out with a bag over their head. The bag is, of course, full of nitrous oxide and no oxygen, and they potentially asphyxiate. But also, the bag usually gets stuck in the mouth and on the nose, making it impossible for air to get through, even if it was available at autopsy, there is typically no findings other than pulmonary edema and congestion. But you can look at the plastic bag and look for condensation, which showed that the individual was breathing when it was on their head. So you'd know they were alive when they were huffing the nitrous oxide. Chronic nitrous oxide abuse can result in other issues like nerve damage, such as numbness, muscle weakness, and impaired coordination. Also, it can cause a vitamin B12 deficiency, which can worsen the neurologic issues, and it also can cause an anemia. The dentist that died had been noticing a decreased ability to do his job and lack of coordination, which was all due to his chronic nitrous oxide abuse. Ultimately, the thing that killed him also caused him to have more stress, and he was using it to try and decrease his stress. So as quite the vicious circle. And it's something to consider that if the stress of your job is so severe that alcohol and drugs are needed to cope, maybe something else needs to be done before it destroys you. The next thing that I commonly see is chlorofluorocarbons, which are also known by their trade name of Freon. Chlorofluorocarbons are organic compounds that contain carbon, chlorine and fluoride. It's a colorless, non flammable liquid and does not occur naturally. It's about four times heavier than air and evaporates rapidly when exposed to air. It dissolves only slightly when mixed with water. Freon's main use is as a refrigerant, especially in air conditioning units. It also is in cleaning products and aerosol propellants, often being present in hairsprays. I have had occasional cases of individuals climbing on top of buildings to access the air conditioning units. They break the Freon line and then place a bag over their head and huff the chlorofluorocarbons as they come out of the broken line. Sometimes they're found dead with the bag over their head, still leaning on the broken line. One interesting finding from chlorofluorocarbon abuse and nitrous oxide, as well as some other inhalants, is there can be frostbite injuries. When the gas comes out of the containers. They can be at a temperature of -40 degrees Fahrenheit, and this can burn the mucous membranes of the nose, tongue, lips and throat and other areas such as the hands and fingers, leaving telltale red burn marks. But these are, of course, incidental. Most individuals who die from huffing chlorofluorocarbons die, just like as the other individuals who huff inhalants, and that is from asphyxiation. But there is a syndrome known as sudden sniffing death syndrome that can occur. It's unpredictable and can happen even on the first use of an inhalant. This syndrome is caused by the fact that the inhalants are cardiac depressants. They cause slowing down of the heartbeat, prolonging the time it takes between contractions of the heart, and this can result in arrhythmias. Inhalants are also believed to sensitize the heart muscle to circulating catecholamines, which are part of the excitatory response this can cause an exaggerated response by the heart muscle. And when a huffer is startled by someone or something, it can lead to a fatal cardiac arrhythmia, known as a ventricular fibrillation, where the heart just quivers instead of pumping blood. Also, this sensitization of the heart can result in arrhythmia when the individual gets done huffing if there is increased physical activity, such as running from the police, or if the individual is experiencing a frightful hallucination or participating in sexual activity. Like with other inhalants, the findings of autopsy with chlorofluorocarbons are completely unremarkable. And establishing a diagnosis is dependent once again on finding evidence of the inhalant in postmortem toxicologics testing. One case of chlorofluorocarbon huffing that I performed an autopsy on had an interesting finding. She was a 30 year old female. She liked to huff hairspray. The particular one she used contained Freon. She would spray it into a rag, put it over her face and huff it to get high on. The last time huffing, she was found dead, face down in the rag, laying on bed. There were multiple cans of hairspray at her side. Her autopsy findings were again nonspecific. She had pulmonary congestion, edema and some cerebral edema, but nothing else. The toxicology came back with chlorofluorocarbons present in the blood and a very, very, very high alcohol level in her vitreous at 800. If you remember in a previous discussion, each can of beer, glass of wine or shot of whiskey can elevate your blood alcohol level to 0.02%. So a blood alcohol of 800 is equivalent to 0.8%, which is incredibly high and would essentially be like drinking 40 cans of beer all in the span of an hour. Her blood level, on the other hand, was completely devoid of alcohol, which makes no sense. Why did this occur? It took some research, but it appeared that the hairspray had also contained a significant amount of alcohol. And when she died and was laying on the rag, the alcohol continued to passively absorb into her eyeball and get in her vitreous and create this false result. The reality was she had not drank alcohol at all and the alcohol level in her eye was just created by this passive absorption of alcohol after she died. As an aside, I did find while researching this, there was a period of time where college students were doing eye shots, or they were putting a shot glass full of alcohol on their eye, trying to get drunk from absorbing through the eye. It is probably one of the more unusual ways I've seen people attempting to get drunk by not drinking it orally. There are other unusual methods for getting drunk, but that's for another show. Her gauze death was acute chlorofluorocarbon toxicity and of course the manner of death was accident and the alcohol had nothing to do with it. to our final case. It was a unique substance that was inhaled. It's in fact the only instance of this that I have seen or experienced in my entire career. The individual was a 17 year old male. He was found floating face down in a bathtub full of hot water. The tub was overflowing and spilled out onto the floor, seeping into the adjacent room. This flooding is how he was discovered. His mother noticed the water flowing out from under the bathroom door. She knocked, no response and she quickly became worried. She began frantically pounding on the door. As the flooding increased, her son continued not to respond. She popped the lock and found him unresponsive, face down in the tub. She pulled him out and called 91 1. She initiated CPR until the paramedics arrived and took over, to no avail. He was pronounced dead at the scene. He was brought to the medical examiner's office and I performed his autopsy the next day. My findings were he was a thin male, appearing to be 17 years of age. He had abrasions on the left side of his face and along his neck. There were areas of skin slippage on his extremities that were more likely than not caused from the hot water of the bathtub, essentially cooking him like a lobster and accelerating the decomposition process. He was also found to have water in his sphenoid sinus, which is a non specific finding in drownings. The sphenoid sinus is located in the anterior cranial fossa at the base of your skull, right around where your eyeballs sit. And when opened and accessed with a needle, you can find water in it. In cases of drowning, it does not exactly mean that the person drowned, but it shows that the body was found in water. He had also moderate edema of his lungs and these findings were nonspecific and there were no other findings of note. His case was made pending to wait for the toxicologic testing to get back. There were two sheriff's officers who attended the autopsy for the purpose of viewing it and finding out what happened. They wanted answers. He had died in a small town in rural Texas, so everyone knew everyone and it was shocking that this 17 year old male had just suddenly died. The sheriffs wanted to figure out what had happened and provide closure to the family. While I was performing the autopsy, the officers and I discussed the findings and they also told me about what they had found at the scene. It appeared that if Seaton had gone into the bathroom and collapsed in the tub, hitting his head, which fit with the autopsy finding of the abrasions of the head and neck, but he was also doing something else while naked and it is not what you're thinking he was huffing something from a 1 liter pop or soda bottle. Before they told me what it was, they said, well Doc, you have to understand he's a weird kid. Keeps to himself. He isn't involved in any clubs at school and he doesn't play sports, especially no football. Just nothing. Then they told me the liter bottle contained a fermented mixture of his urine and feces. This was the substance that he was huffing. The concoction is known as jinkum. Jinkum is a hallucinogenic inhalant created by fermenting feces in urine. The drug supposedly originated in Africa, where it was reported by the BBC in 1991 that street children in Zambia were inhaling fermentant sewage. And of course, because it was the late 90s and early 2000s, this information became available on the Internet. Supposedly kids in America began claiming they were huffing jenkum as well. And for a short period of time, many local news reports and TV stations picked this up and ran stories on it, advising parents to make sure their kids were not huffing their own feces, telling them to check their kids breath before they went to bed. As it turned out, jinkum use is largely a myth and its use in the United States was almost entirely a hoax, with most of the Internet sites being unsure if it was real or not. But as I have learned so many times, people do strange things and my case was definitely a real case of jinxum use. Let's discuss jinkum a little more in depth. To make jinxum, fecal matter and urine are placed in a bottle or jar and covered most commonly with a bloom. It's then placed in a sunny area for several hours or days until it ferments. The contents will separate and release a gas which is captured in the balloon. Inhaling the gas is said to have a euphoric high similar to ingesting cocaine, but with strong hallucinations. Once ingested, the onset of the high takes approximately 10 seconds, with most severe hallucinations happening in approximately 20 minutes. The literature states after inhaling, the subject often will pass out and then wake up and achieve a magical hallucinogenic state within seconds of regaining consciousness. The high has been described by subjects as feeling as being out of it, and it may last for several hours. Most of the literature though reports jenkum use to again be a myth, but in this particular case it was definitely real. The officer showed me pictures of his bizarre concoction that they found at the house. It was in a green Plastic bottle. It appeared to have urine and feces sitting in it. They were separated. They didn't find any balloons at the scene, but he could have been just huffing it directly from the bottle. It definitely looked real to me and not a hoax. With no findings at the autopsy, like I said, the case was made pending. I ran toxologic tests expecting to find one of the usual suspects, especially methamphetamine, which seems to be everywhere in rural America. And when it doesn't make sense, it's usually meth. Definitely, when someone's huffing their own feces, that really doesn't make sense. When his toxologic testing came back, his femoral blood was positive for a substance known as thiosulfate, which was well within a fatal range. Thiosulfate is produced by the metabolism of hydrogen sulfide. Hydrogen sulfide is a colorless flammable gas produced by decaying organic matter. It's also present in feces, and in high enough concentrations, it's extremely toxic and can result in sudden unconsciousness. It is produced by anaerobic bacteria in the colon as they break down sulfur containing compounds in certain food like eggs, meat, garlic, onions, and cruciferous vegetables, which include radishes, turnips, and Brussels sprouts. The production of the gas is part of the normal gut digestion process. Hydrogen sulfide is one of the volatile sulfur compounds that contributes to the smell of flatulence and feces. By fermenting his own feces and urine, he probably created hydrogen sulfide more than anything else. And he was basically huffing that which caused him to pass out, and he fell into the bathtub full of water and drowned. He had no other findings other than the thiosulfate in his blood, so his cause of death was made. Hydrogen sulfide gas toxicity was a significant contributing factor of drowning. His manner of death was accent. It was definitely a weird way to go out, but I don't think it had anything to do with his lack of sports aptitude. And it was probably more of his curiosity and boredom. One could almost blame the Internet for his death. And that brings us to the end of the episode.
