Gabriel House and Fall river, according to Massachusetts Executive Office of Elder affairs submitted information on 26 incidents that could have impacted residents welfare outside the required 24 hour reporting period that happened between May and October 2020. There were also issues with missing documents and how medicines were administered as it relates to the deadly fire this week there have been questions about staffing levels at the time and if there was a plan in place to increase the number of trained workers in case of emergencies.
