In this episode, we break patient outcomes down into a simple but powerful formula:Patient Outcomes = Skill × TimeYou need:The skill to understand what’s actually driving someone’s symptomsAnd the time for healing, behavior change, tissue adaptation, relationship-building, and clinical reasoning to unfoldMiss either one — and outcomes suffer. We talk about:Why outcomes can’t be separated from both clinician skill and available timeHow testing your hypotheses requires both technical ability and space to observe changeWhy healing, strength, and behavior change don’t happen on insurance-driven timelinesHow different practice models dramatically affect the “time” variableThis conversation was sparked by recent rhetoric criticizing cash-based care — and instead of taking the bait, we zoomed out and asked a better question:What actually creates great outcomes? This episode sets the foundation for future discussions about systems, incentives, and why clinicians often feel frustrated despite doing their best work.🎧 If you care about results — for patients and clinicians — this one matters.About UsNicole and Jesse Cozean founded Pelvic PT Rising to provide clinical and business resources to physical therapists to change the way we treat pelvic health. In this episode, we break down how money actually moves in the insurance-based PT and OT world, why so many clinicians and clinic owners feel financially stuck, and how the landscape has quietly but dramatically changed over the last 20 years. We cover:How insurance companies make money — and why they’re incentivized to pay out as little as possibleThe reality of declining reimbursement and rising administrative costsWhy clinic owners feel squeezed from both sidesWhy clinicians deserve raises — and why insurance doesn’t support that realityHow this system unintentionally pushes clinics toward lower-quality care modelsMost importantly, we talk about why the rise of cash-based care isn’t a trend — it’s a correction.Cash-based models:Allow small, independent clinics to survive and thrive againCreate real earning upside for skilled cliniciansRestore transparency and accountability in careExist in every healthcare system — including countries with universal healthcareThis episode isn’t about villainizing anyone. It’s about understanding the system we’re operating in — so we can make better, more informed decisions about our careers, our clinics, and our patients.🎧 Whether you’re an employee, a practice owner, or somewhere in between, this is required context for the modern pelvic health landscape.About UsNicole and Jesse Cozean founded Pelvic PT Rising to provide clinical and business resources to physical therapists to change the way we treat pelvic health. Wondering about the science behind red light therapy and whether it's 'a thing' in pelvic rehab? So were we! So we brought Liz Frey on the podcast to answer all of our questions about photobiomodulation and it's uses in pelvic therapy. We'll talk about the basic science behind red light therapy and what the research says on how it can help. We'll also talk about the development of the Fringe Red Light Want, discuss how Liz uses red light therapy with her own pelvic patients and how they can be an adjunct to pelvic therapy. Can't wait for you to hear about the possibilities of this technology!Full Disclosure: I provided (unpaid) advice to Fringe as they were developing their red light pelvic wand to help make it more usable and approachable for patients. Fringe has also sponsored PelviCon and I have an affiliate link with them for anyone who orders their products (and also gets people 25% off when they order before the end of the year!).Fringe and Liz FreyFringe (https://fringeheals.com/ref/168/) is a health company focused on evidence-based use of red light therapy. They have a red-light wrap, a face mask and a pelvic wand all using the same technology. From now until the end of the year, you can get 25% off ANY Fringe product (even those out of stock!) using the code NICOLE25.Liz Frey is the Pelvic Health Medical Director at Fringe, as well as an educator and pelvic health clinic owner herself. Why do we expect to be great business owners without any training at all? We don't think that way about our clinical skills. In fact, we all spent 7+ years in school, countless clinical con-ed hours and often hundreds of thousands of dollars to become good clinicians. And yet, most of us have never invested anything in ourselves as business owners. Yet we expect to have immediate success - no wonder 90% of us aren't happy with what we're making financially in business! There's a 'compound interest' in becoming a better business owner. A small improvement or investment now grows and grows over the years and decades we own our practice. If we develop a front desk script now, we might use it for thousands of patients. Our call logs, budget templates, 'last 5' for clinical buy-in - all of these skills or habits will last for years. These investments in the business will pay off over the life of your practice. Yet we don't approach business with the same dedication we do our clinical practice. As we said in a recent podcast 'sode, "If we invested 1% of the time and money in business ownership that we did to become great clinicians, 90% of us wouldn't be unhappy with what we make in the practice!Business Accelerator Program - Closes Friday!Registration for the January Cohort of the Accelerator Program closes on Friday (12/20)!! If you aren't feeling satisfied with your income, you aren't alone. More than 9 in 10 business owners say they aren't happy with what they are making from the business.....but why? That's what today's 'sode is all about. We may have heard how 'easy' it is. Or think everyone is successful because that's what we see online. But we're going to lay out the truth for you when it comes to business finance!1) Expectations - A lot of business owners 'just do the math'. If they saw the same number of patients they were seeing elsewhere, but kept all the money....not taking into account the challenges of marketing, administration, and fulfillment. They don't know their minimums and keep moving goalposts.2) Such Hard Work - Many business owners feel like what they make isn't commensurate with the work, stress and hours they are putting into the practice.3) Aren't Making Money - Some business owners really are struggling with revenue. The phones aren't ringing, they aren't able to convert the leads they get, and the patients who do come in tend to drop off or space our visits.4) They Aren't Getting Help! - These numbers change dramatically for those getting help! We've all spent multiple years and hundreds of thousands of dollars to become excellent clinically, but haven't invested in ourselves as business owners.Business Accelerator Program - Now Open!Registration for the January Cohort of the Accelerator Program is now open! Why is it so important to get all three of these pillars - Marketing & Sales, Business Systems, and Clinical Excellence - right for a well-running business? In this 'sode we discuss the three main pillars of business. If only one or two of these are working well, the business is going to take a lot of effort and always feel precarious.Marketing & Sales - If you don't have a consistent source of leads and are always worried about getting the phone to ring, marketing your practice is vital. We need a systematic plan for marketing to avoid dips and the 'rollercoaster' of business.Business Systems - If you're feeling overwhelmed or burned out, it's usually because you're lacking systems. We often make up for poor systems with more work and late nights, but that's not sustainable.Clinical Excellence - If you're having patients drop off, space out their visits, or stop coming in because 'it's expensive', we aren't getting the clinical buy-in necessary. When all three of these are working well, the business gets much easier to run, grows faster and is actually fun to work in!Accelerator Program - Registration Opens Dec 12th!If you know one - or more - of these areas needs work, consider joining us for the January cohort of the Business Accelerator Program!
