It needs to be prioritized because you would think that if something was homogeneous or bilaterally swollen, then it's probably less of an issue than something that's unilaterally swollen, meaning one sided. So, you know, obviously you don't want to. If that issue were to get worse, you don't want to have any compression on your airway, you don't want to have hoarseness, trouble swallowing, etc. So if I were you, I would get that ultrasound right away because you want to see what's what. The reasoning for the extra tissue is, is it a cyst? Is it a nodule? Is it something that's not supposed to be there? Is it an autoimmune thing? So it's not something I would put off to the wayside. I would, I would expedite getting better pictures so that you can make decisions about what you need to do. But I don't know if there's anything

So when I went to medical school, I had to, like, I feel like part of my brain just dumped information from the past and had to make like a whole new vocabulary. Because every drug has two names, and then you have to memorize what the drug does, both of the names, and they're constantly coming out with new drugs. So it is very difficult. So sometimes if someone comes in from a specialist and has a new drug, I'm like, oh, my gosh, I've never seen this and have to sit and look it up, read about it. So a lot of time is.

The show scrubs is definitely 150% written by someone who was in medicine, because every single character in that show I can equate to someone I've met in my life, and they probably still work at the hospital that I used to work at. So you use laughter as a way to get through the hard things, which I think a lot of humans do, but. But it is a very scarring profession for all of the people that are caregivers because those memories are with you and you got to break out of it somehow.

I wanted to ask regarding scrubs though, like, do surgeons and doctors, do they like draw a line between each other? The way they show on the TV show, is there this kind of like, I'm a surgeon camp and I'm a doctor camp and they eat with the other surgeons and the doctors eat with other doctors. Is there this kind of like distance between them?

Yeah, there are clicky because. And it's not in a bad way, but like the internal, the medicine people are more than nerdy people because we're doing all the behind the scenes work. We're making sure that the patients are staying alive during the night after they are operated on, keeping them alive so they don't die before the surgeon gets there to do the surgery. But we tend to get pushed into the nerdy scene because we're not doing, we're not, you know, scrubbing in and cutting people open and taking things out. So we're kind of like.

Yeah. And the surgeons, most of them do like think they're like, shit doesn't stink because they are beloved in the hospital and they walk around with their little scrub caps on and they, you know, they want, they, they get a lot of kudos because they do do a hard surgery is hard. It's a way longer residency than I had to do. So there is much respect for them. But they tend to be a little rough around the edges and pompous at times. But you really can't let that scare you because, you know, when it comes down to it, once they're out of the OR and you're in charge again, you're still keeping that patient alive. So much respect to them for doing that. But at the same time, is that

They do the same internship as we all do and then they leave us and they go to do their five year surgical residency.

Oh, gosh. It's a hard question because you have a lot of good moments. I think that when I would say there's not one experience, but I think when you run, I think the one thing that I have always, like, can remember is when someone would, like, code or stop basically trying to die, their heart would stop or they'd stop breathing. The times where I was successful in bringing them back, I feel like that was, like.

A lot. Because I worked in the ICU for 10 years, and those are the sickest of the sick.

And have you ever met anybody that was like, what the fuck? I was dead. No. And you fucked it up on me.

And a lot of people that you save don't remember because they're out of it. They're so sick, they have no memories.

Not. No. I haven't had anyone. I haven't had anyone tell me anything like that. But I think those particular moments are the ones that you. The reason that you do what you do. Like, you know, they always stick with you.

Like, so when you. So usually the nurse. The nurse does that for you when you run the coach, because the nurses are your team. So you stand at the edge of the bed and you watch the monitor, and you tell. And it's protocol. You have to tell everybody what to do. So this nurse does this. You push this drug. You do this, and then someone will probably be doing cpr. And then you're watching the monitor, assessing the rhythm, and then giving the drug as you see fit. So, like, one time, it was, like, as simple as, like, a woman had. She just was sitting up, and she just went out, and he looked at the monitor. It was a specific arrhythmia. I said to the nurse, like, give this drug. I gave the drug. Boop. Right back. That's like a really baby aspirin. Tic tac. Boop. No, but it was like, those are the experiences that are really rewarding, and those are the ones you remember, but you don't have one moment that sticks out.

Do you ever do the thing with the compressions?
Dr. D
You do sometimes, yeah. Like, you know, if you don't have any help and you're by yourself, but usually people come to your aid and help you. I know one time we had the paddles, and we almost all died because it was my first code I ever ran. And we went. So we had to take the patient from the cardiac floor to the ICU in the middle of the night. So all the nurses and myself shoved the patient in the elevator and tried to get down to the bottom floor, which her heart stopped again. So we had the shocker again. But then I realized we're inside a metal box with the paddles. We're all gonna get electrocuted. So the nurse goes to turn the machine on. And I was like. And I don't know, maybe we wouldn't have got electrocuted, but I thought of it, and I'm like, stop. Everybody off the elevator. So we got off the service elevator, went out into the hallway, shocked the patient, and then went back in the elevator and went down. But we all.

Those are the people I'm not sad to see go. Because there's only so much verbal abuse that you can take in one day before you're like, I don't feel like doing this.

Do you find that some nurses are too big for their britches? I always hear about nurses being like, we do all the work. The doctors don't do shit.
Dr. D
So there's usually one or two that you don't get along with, and that's personality conflicts, but they're like your lifeline. Like, because I would just. I, I. When I would be on call by myself for the night in the icu, I would just peek in the door and see who was working that night. And if I saw the. Like, some of the. That were working, I'm like, no. But then if I saw all my girls that I liked or guys. There wasn't that many guys that were nurses at the time, I would be like, yes, my favorite nurses are here. And it's going to be so good because we all get along. Then there's the ones that want to fight you for some reason, and you're like, come on, can we just get along so this night goes faster.

So you mentioned that that first time you were. It was your. You were in charge of somebody, the code. So, like, that shift, like, was there. Like, you. You knew that sooner or later it's going to happen. Like, you're. Sooner or later this is going to happen, and you're going to have to be the one to, like, start barking out all the orders. Is that kind of like just like waiting for that moment? Because it's going inevitably it's going to happen. Like, do you rather have it happen right away or is the longer it goes, the more you get in your head?

I mean, every code blue in the hospital, you have to go to. So it could happen five times a day, could happen zero times. It really depends on. It's literally luck of the draw. But I'd rather get it over early because the later it gets in the day, no one's around.

It. Actually, that's funny you say that. The first night I was on call by myself, it happened, like, with. Within one hour of the shift. And that was the elevator incident. I'll never forget that because I was like, man, like one hour, just already one hour into the shift and I already have to do this. But it's part of the job. And fortunately, you've a lot of people there to help you. The nurses know what to do. But it is kind of scary being the one in charge of it all. You can't slip up.

Yeah. So after a while, if you know, because you know you have to say, enough's enough, like, stop.

room all the time? Yeah. So can I. This is why I used to be so shy and scared. One night in the middle of the night, 40 family members came into the. Into the family room because one of them, a mother of the family, the matron of family, was ill, and the nurse came in and said, you need to. I need you to go out and talk to these people. So I'm like, oh, it's probably, you know, like, one or two people, son and daughter. 40 people who. I don't know, crying, eating food. Everyone's hanging out in this family room, and I'm like. Like, oh, my God. Like, I have to tell them. I have to tell them that it's, like, really not going well. Like, once you get maxed out on four medications and you're not getting any better, what's left? Magic, you know? So. Believe in magic. So I just said, listen, it's not going well. We're doing everything we can. I lay it out on the table. I will continue to try for the rest of the night as hard as I can, but I can't promise you that this is gonna have a good outcome. I apologize in advance if it doesn't, but I will keep you posted every hour. But talking in front of 40 people who are all edged up because they're. Or not edged up. I don't know what the word is. Like, amped up because they're worried is like, you know, you're sweating through your scrubs because you're like, oh, my God. And I'm like. That's when you get.
