Welcome to Wellness on Mass. I'm Dr. Nicole Safire and this is your weekly rundown. Well, unless you've been living in a hole, you've probably heard former President Biden has been diagnosed with an aggressive late stage prostate cancer. We don't have all the information and there's still a lot of questions, but some people are starting to wonder, was this cancer diagnosis missed by the White House physician, or did they know about the cancer and they withheld the diagnosis from the American public? It's not really a stretch, considering how they withheld the cognitive decline over the course of his administration. So I decided to do a deep dive into his cancer screening history, but also his prostate health history as well. This is what I've come up with. First, I should note that the Biden spokesperson came out and said the last time that they screened for prostate cancer in former President Biden was 2014, and therefore he wasn't being screened, so therefore they didn't miss it. Well, while we do recommend stopping cancer screenings, you know, after a certain age, there's not an immediate cutoff. But the reality is you kind of look at someone's life expectancy and decide whether or not you want to keep screening for cancer if there really is a benefit to it. Why this doesn't pass my SNF test is the fact that, okay, so they said that they stopped screening him for prostate cancer in 2014, but yet in 2021, he had a colonoscopy for colon cancer screening. And by the way, prostate cancer is more common in older men than colon cancer. So why are you screening him for colon cancer but not prostate cancer? That doesn't make sense to me. He was also getting annual skin checks for skin cancer screening. So he was still doing cancer screening up until he was 82. So don't tell me you decided to stop screening him in 2014 before he was even president. I don't buy that. In 2019, Biden's physician said that he was being treated for BPH, also known as benign prostatic hyperplasia. This is completely benign. It's when cells kind of grow in your prostate, but you haven't. It's not cancerous. It's all completely benign, but it can raise your PSA levels. So how did they know he had bph? Well, he probably had some symptoms, Commonly urinary frequency getting up during the night. Or maybe they saw it on a PSA screening. They saw that his PSA level went up. Well, when you have a high PSA level, you obviously want to make sure that there's no cancer there. So didn't he have a workup? Did he have an MRI of his prostate? Did they do a physical exam? Did they check for nodules at that time? I'm going to say they probably did. I mean, he was running for president. Of course they did. So that tells me in 2019, he probably didn't have prostate cancer, because I'm going to assume that he got the full workup because he's vip. Let's be honest. They said he was being treated for it. How was he being treated? He was probably given a medication. Those medications that treat BPH can lower your PSA levels. So if he was getting subsequent PSA levels after that screening for prostate cancer, it's possible that that medication he was taking gave false negatives, meaning cancer was starting to grow. But his PSA levels were low from the medication he was on for the bph, so. So they didn't see the rise in psa. So it's possible they missed it. He probably wasn't getting a physical exam to check for those nodules on the prostate, because any man can tell you no one likes to have this physical exam. It's a rectal exam and it's uncomfortable. So a lot of men opt out of having the physical exam and just going with the blood test. But again, he was on a medication that could lower his PSA level, giving him a false negative. So it is possible that the cancer began growing and they missed it. So now the other question is, did they know about his cancer and they just didn't disclose it to the American public? Well, that's also possible. It is highly likely that the cancer developed while he was in the White House. I doubt it was there in 2019, but I bet it was there sometime in the White House. When did it start? I don't know. When did it metastasize to the bones? I don't know either. But I can tell you that he had multiple MRIs of his spine and hip hop throughout the course of his presidency. Now, they never said specifically he had those MRIs. They just said that his degenerative change of the spine stayed the same, but there was no nerve root compression. And the way that we look for nerve root compression is with an mri. If he had metastatic lesions from prostate cancer in his spine or in his hip and he had these MRIs, we would have seen them, or else I would hope that radiologist picked it up, or else I'm going to have a serious conversation with who's reading the President's films. So my guess is he probably didn't have metastatic disease as late as 2024, because that's the last time we heard about his degenerative disease of his spine, February 2024. But imaging that he did have after that, he got Covid in July 2024. Now, while a CT chest is not indicated for people who get Covid. He probably had a CT chest because again, he was the president and you lay out the red carpet and in fact, he had a CT chest in the past, as did President Trump, when just to make sure that the lungs look okay. Did they see something on that CT chest? Because the CT chest includes the thoracic spine. Did they see any lesions in his spine that would indicate an aggressive or an advanced cancer diagnosis? He dropped out of the presidential race about a week later, so maybe that's the time that they had an inclination that there was advanced disease going on. I don't know. Obviously we don't have that information. All we heard was that in the last week he presented with symptoms, they found a hardened nodule, and then quickly they diagnosed him with not only an aggressive prostate cancer, but one that metastasized to the bone. They didn't tell us how they diagnosed that, so I don't know. So my take is I doubt he had prostate cancer in 2019 because he had the BPH workup at that time and they probably would have or should have found prostate cancer at that time. It's likely that it began forming while he was in the White House. It's quite aggressive, so it's probably been within the last couple of years. And it's unlikely he had metastatic disease of the spine, at least through his MRIs from February 2024. I do wonder if they saw something July 2024. But at the end of the day, former President Biden has an aggressive, very late stage prostate cancer. And good news is there are some treatments for him, but he'll never be cured from it. All we can hope is that he isn't suffering from the intense bone pain that can come from it and that he's able to take some time to relax, spend it with his family. And I just hope that they are being fully transparent with the American people, unlike how they were with his cognitive decline.
