In this episode, I break down why that conversation is usually way more nuanced than most people realize. I walk through what hematocrit actually is, why testosterone raises it, and why a higher number on paper does not automatically mean something is dangerous. We talk about how hydration status, sauna use, cardio, lab timing, sleep apnea, insulin resistance, inflammation, and metabolic health all change what that number really means in the real world. I also explain why I think routine blood donation is often overused in men on TRT and how it can create its own problems by driving down iron and ferritin. Most importantly, I give you a practical framework for when I think hematocrit is usually a non-issue, when it deserves more attention, and what I would adjust before ever jumping straight to phlebotomy. This is a deeper dive into one of the most misunderstood topics in testosterone therapy, and my goal is to help you think about it with a much better framework.

I break down what DADA actually is, how it works as a metabolic modulator, and why I view it as one of the most underrated performance-enhancing tools in this category. To me, it feels very similar to MOTS-c in practice, but in some cases it may even give a stronger performance edge.I walk through the mechanism behind DADA, including how it affects PDK and PDH to improve mitochondrial energy production, glucose oxidation, ATP output, and metabolic flexibility. We talk about the wild fact that DSIP can cross the blood–brain barrier, and why that makes nasal spray formulations surprisingly effective in real-world use.From there, we get into mechanisms: DSIP’s ability to lower ACTH and cortisol, act as a “stress-limiting” factor, nudge growth hormone a bit, modulate pain pathways, and influence neurotransmitters like Substance P. I break down why I see it as a multi-system modulator that gently rebalances the body’s own sleep circuitry instead of sedating you like Ambien.We cover the human data from the 1980s – the insomnia trials where DSIP normalized sleep architecture, the European study using DSIP in alcohol and opiate withdrawal with surprisingly high success rates, and a small pilot in chronic pain and tinnitus where most patients had meaningful relief.Then I share how I actually use it: my preferred subcutaneous dosing range of 100–500 mcg, why I think 2–3 hours before bed on an empty-ish stomach works best, and how I’m planning to use DSIP more aggressively for jet lag and circadian realignment when I travel.
